Applicants are required to provide identification when applying for NSW security and tattoo industry licences.

Security Industry
Applicants must provide copies of two documents from the following list, one of which must contain your photograph.
- Australian Passport clearly showing your name, date of birth, photograph, passport number and expiry date (may be expired within last 2 years)
- Australian Birth Certificate (not an extract)
- Australian Citizenship Certificate (front and back page)
- Australian Driver Licence
- Australian Marriage Certificate
- Health Card
- Immigration Card (Immicard)
- Medicare Card
- Pensioner Concession Card
- Senior’s Health Card
- Health Care Card
- Australian Certificate of Registration by Descent
- Foreign Passport clearly showing your name, date of birth, photograph, passport number, expiry date and country of issue (an original certified copy only)
- Veteran Seniors Health Card (an original certified copy only)
- Veteran Gold Card (an original certified copy only)
- Veteran Pension Concession Card (an original certified copy only)
- NSW Photo Card / Proof of Age Card (an original certified copy only)
- Aviation Security Identification Card
- Maritime Security Identification Card
Tattoo Industry
Tattoo licence and permit applicants must provide copies of two documents from the following list, one of which must contain your photograph. Close Associates must provide three forms of identification from the following list.
- Australian Passport clearly showing your name, date of birth, photograph, passport number and expiry date (may be expired within last 2 years)
- Australian Birth Certificate (not an extract)

Secondary Identification Documents (tattoo industry close associates only)
Some tattoo industry close associate applicants may be unable to provide three forms of identification from the primary list. In that instance, applicants must provide two forms of identification from the primary list and one certified copy of a document from the secondary list below containing their full name and either their current residential address, photograph or date of birth.
- Photo identification card issued for Australian regulatory purposes (e.g. Maritime Security Identification Card, security industry licence etc)
- Government employee identification card
- Defence Force identity card
- Property lease agreement (current address)
- Identification card issued to a student at a tertiary education institution
- Rating authority (e.g. land rates)
- Utility bill (e.g. electricity, gas, telephone)
- Financial institution notice issued within previous six months
- Land title document
- Reference from Indigenous organisation
