Privacy Collection Notice and Consent
Officers that left the NSW Police Force after 5 February 2020 due to a work-related injury or illness are encouraged to complete this form to assess whether they are eligible for Australian Honours.
By completing the form, the NSW Police Force will have the required consent to review the eligibility of the applicant for an Award, being the National Medal or the clasp the Governor-General of Australia.
Providing the requested information is voluntary. However, if you choose not to provide us with your information the Department and/or the Australian Honours and Awards Secretariat will be unable to assess your application under the National Medal Regulations 2011.
